Welcome to SDR Educational Consultants' Application Page for previous customers. From this page, you may request additional copies of a previous evaluation/translation, renew your previous file for an additional five (5) years, and/or upgrade your previous evaluation by adding additional educational documents and information. The application process the services is simple:

Complete the PDF application form available for download below;

Submit your application, payment and any additional documents (if necessary) to SDR.
You may submit materials via email, mail or office visitation. SDR's address is: 2600 N. Gessner, Suite 267, Houston TX, 77080, USA.
Personal consultations at the SDR office are available 9:00 A.M. - 5:00 P.M. Monday through Friday. Walk-Ins are welcome.

General Tip:
Please do not use the Browser''s BACK button instead use the navigation buttons "Previous , Continue" as otherwise it might lead to errors on your application. Be sure to press"SUBMIT" after you enter any data.

Application requires enabling of Javascript (Internet explorer v8.0):

Open Internet Explorer.

Click on the Tools from the top menu.

Select Internet Options in the menu.

Click on the Privacy tab near the top of the window.

Click on the Default button of the window.

Move the slider so that it is on one of the levels below Medium High (including Medium, Low, Accept All Cookies).

Save changes by clicking OK.
