SDR Educational Consultants has set this page up for FosterQuan, LLP. FosterQuan staff may request services from SDR and upload scanned PDF's of documents through this online portal.

The process is simple:

1. Begin by typing in the "Captcha" characters at the bottom of this page for security.
2. Quickly enter the required client information (required fields have a red asterisk, optional fields don't).
3. Select the service(s) you are requesting from SDR.
4. Upload scanned PDF's of the documents to be reviewed, evaluated or translated. 

You may choose to mail or email documents to SDR instead of uploading them through this portal.

Personal consultations at the SDR office are available 9:00 A.M. - 5:00 P.M. Monday through Friday. Walk-Ins are welcome.

General Tips:

• You must complete this application in order for your information to be saved.
• If you make a mistake in the information you submit, please continue with the application, and you will be able to change your information at the end of the application.
• Please do not use the Browser''s BACK button instead use the navigation buttons "Previous , Continue" as otherwise it might lead to errors on your application. Be sure to press "SUBMIT" after you enter any data.

Application requires enabling of Javascript (Internet explorer v8.0): 

• Open Internet Explorer.
• Click on the Tools from the top menu.
• Select Internet Options in the menu.
• Click on the Privacy tab near the top of the window.
• Click on the Default button of the window.
• Move the slider so that it is on one of the levels below Medium High (including Medium, Low, Accept All Cookies).
• Save changes by clicking OK.
